PCYC ILLAWARRA: GET ACTIVE REFERRAL PROGRAM 2018

                                                                                                                                                                                               

Program Initiative:

Address and engage youth 12-24year in positive programs, activities

Links and connections to appropriate service providers for health, wellbeing and support

Intervention/ prevention and support pathways for young people whom require support with health and wellbeing

Phase 1. 2016:

In 2016 PCYC Volunteer the Now Dr. Kevin Rourke an active member and volunteer with the PCYC Shoalhaven developed and implemented the Pilot Program Youth Boxfit program within the region. Program objective was a research discussion piece in partnership with PCYC Shoalhaven and University of Wollongong School of Medicine and the link between Youth mental health and Physical activity and how Physical activity can positively impact and improve a young person’s mental health.

Throughout the program the participants were surveyed and all research evaluated and the outcome that 80% of the participants reported increased self-esteem after engaging in the

Phase 2. 2017

After the evaluation period in house revision and discussion as to an additional program and transitional supports to continue to engage and support young people. GP referral Program developed- the program is based on the Mental health Care plan – where a person/s can present and be assessed and receive an appropriate referral- as an step prior to this the GP Referral Program aims to be an intervention/ prevention step that if a young person 12-24years present to their GP identifying any health or wellbeing concern as a form of first point of referral is to the PCYC to actively engage in Fitness/ activity and Youth supports.

Alongside PCYC Direct service delivery and engagement Dr. Kevin Rourke and his research team are completing surveys and assessments on the referred clients and will present a research paper at the end of the program DEC 2017 to a medical journal.

2018- Cross Regional ROLL OUT- ILLAWARRA

PCYC SHL, Fundraising efforts is to allow the Program to be piloted and rolled out across regions within the Illawarra, allowing Young people 12-24years FREE Access to clubs, programs and supports. Lake Illawarra, Wollongong and Bulli PCYC’s. The Illawarra Pilot has collaborated SHL Practitioners Feedback into adjusting the program to ensure it is more accessible, responsive and available: The Referral process is now inclusive of ALL health practitioners including school counsellors and eligibility is inclusive of any presenting Health issue from formalised mental health, obesity to socialisation and self-esteem.  The referral process- is altered to remove barriers, the referral organisation is to electronically Refer client either utilising the Referral pads (scanned) or email providing the required information and PCYC will make contact with the Client/ client also encouraged to attend club on own accord once referral info sent via health practitioner.

2018 and Beyond:

GP referral/ Get Active Referral Program is to be evaluated and presented to Medicare NSW – as an partner / funded provider for young people across the whole of NSW to support and address young people in health wellbeing as a primary referral point from GP’s and Health and wellbeing specialists.
